Payments & Insurance
Cornerstone Preventative Health & Wellness is a private-pay practice. We do not accept or bill insurance, including Medicare or Medicaid.
Payment is due at the time of booking or service.

Upon request, we can provide a superbill for you to submit for possible reimbursement. However, we make no guarantees of reimbursement.
It is your responsibility to understand your insurance plan and coverage.

Cancellations & Refunds
We require 24 hours’ notice to cancel or reschedule a telehealth appointment. Late cancellations or missed appointments may be charged a cancellation fee, as outlined during scheduling.
Refunds are not issued for completed consultations or missed appointments.
